RADIOLOGIC SIGNS OF SIGMOID VOLVULUS
1. COFFEE BEAN SIGN
Modality: Plain abdominal X-ray
Description: A massively dilated loop of colon with apposed medial walls and a central cleft, resembling a coffee bean. The apex often points toward the right upper quadrant.
2. BIRD Contrast enema (barium or water-soluble)
Description: Tapering of contrast at the site of the volvulus, where the twisted colon narrows into a sharp, beak-like appearance.
3. OMEGA LOOP SIGN
Modality: Abdominal X-ray
Description: A distended loop of sigmoid colon forms an omega (Ω)-shaped shadow.
4. BENT INNER TUBE SIGN
Modality: Abdominal X-ray
Description: Another term similar to the coffee bean sign; describes the appearance of a closed loop of distended bowel.
5. WHIRLPOOL SIGN
Modality: CT scan or sometimes ultrasound
Description: Swirling appearance of the twisted mesentery and vessels at the root of the sigmoid mesocolon.
6. INVERTED U SIGN
Modality: Abdominal X-ray
Description: The dilated sigmoid colon forms an inverted U shape, often centrally or left-sided.
7. FRIMANN-DAHL SIGN
Modality: Abdominal X-ray
Description: Three dense lines representing the twisted mesenteric folds converging toward the site of torsion.
